Medication mismanagement is a critical issue for community-dwelling older adults with chronic conditions. In 2017, more than 50% of Ontarians aged 75 years and older were engaged in the concurrent use of five or more medications. The implementation of a nurse-pharmacist collaboration (NPC) intervention aims to address these challenges by promoting safe medication practices among this vulnerable population. This intervention seeks to enhance medication safety, empower patients, and improve overall health outcomes. Insights gained will offer valuable perspectives on integrating nursing and pharmaceutical care.
Evaluating this NPC intervention will highlight its potential to improve medication safety and empower older adults with chronic conditions. The insights gained will enhance interdisciplinary collaboration, inform healthcare policies, and guide the development of effective strategies for managing medications in the community setting, ultimately leading to better health outcomes for older adults.
