The overall objective of this study is to develop a rapid response notification order to the addiction medicine team in order to initiate a substance abuse avoidance plan in the patient prior to their discharge from the hospital. We will develop and test an identification tool that will be a checklist of potential presentations for a person with a substance abuse problem. We plan to use a retrospective chart review to determine the baseline clinical metrics for patients presenting to the emergency department who are suspected of having a substance use disorder, including treatment outcomes, patient disposition, and intervention outcome of the addiction medicine services. Furthermore, we hope to identify opportunities for expanded scope of the rapid response order sheet and applicability across other hospital departments and locations.
